PLEASE DO NOT MAIL IN PAYMENT I hereby authorize (his or her) participation in the Bard Music Camp at Bard College for June and July 2025. I further release Bard College, its directors, officers, employees, students, agents, representatives or affiliates, from any and all liability caused in whole or in part by anyone affiliated with Bard College or from any other cause whatsoever, which results in bodily injury or personal injury suffered by above named minor participant and for any loss or loss of use or damage to personal property owned by said participant or family member, or any other damage or injury, which may result due to the above named minor’s visit or use of Bard College facilities during this activity. I further understand that all Bard College employees including faculty, staff and volunteers involved in said visitation will exercise reasonable care and act responsibly while conducting activities of said visitation. I further understand that Bard College does not provide Health Insurance for students during their participation in the Bard Music Camp. Health Insurance is the sole responsibility of the child’s parent(s) or guardian(s).

Policies and Procedure: Please Read and Check (required): WITHDRAWAL AND REFUND POLICY Tuition refunds will be given at the following rates: • Before May 1, 2025, students will receive a FULL refund (minus the application fee) • From May 1 to June 1, 2025, students will receive a 50% tuition refund • After June 1, 2025, NO REFUNDS will be given *Please Note: Application Fee is not Refundable at any time of withdrawal. If you wish to withdraw, you must notify the Camp directors by email at [email protected]. The effective date of withdrawal is the date that the written notice is received. Students receiving financial aid forfeit their aid upon withdrawal. STUDENT ILLNESS To avoid the spread of illness and infection, we ask that students avoid coming to Bard Music Camp with the following symptoms: Fever, diarrhea, or vomiting in the previous 24-hour period Extreme coughing Impetigo Conjunctivitis (pink eye) Chicken pox - until all lesions have dried and crusted Strep Throat - until at least 24 hours of treatment Infection with head lice, scabies, or pinworms - until at least 24 hours of treatment Skin rashes Measles, mumps, rubella, pertussis, hib, and tuberculosis If students come to Bard Music Camp with signs of the above illnesses or other potentially contagious conditions, we reserve the right to refuse their participation for that day. PHOTO RELEASE By enrolling in Bard Music Camp, the student or parent/guardian (for students under 18) authorizes the making and use of any photos, videos or sound recordings during his/her participation in classes, lessons, or performances for any purpose without compensation to the student or parent/guardian.
